SWIMMING CONSENT FORM

Please fill this consent form for each child

Return of Year 3 and 4 swimming consent letters by 7th Feb 2025

to participate in swimming lessons provided by Maverick Sport LTD. Each session will last for approximately 30 minutes.

By the end of KS2 your child should be able to;

Swim competently, confidently and proficiently over a distance of at least 25 metres
Use a range of strokes effectively – g. front crawl, backstroke and breaststroke
Perform safe self-rescue in different water-based situations

As part of your child’s education they will be undertaking swimming lessons this year where these skills will be covered. It is important that the swimming teacher /instructor has the following information concerning your child;

It will be down to the swimming teachers professional discretion in regards to any flotation devices your child may or may not need.

Chemicals in the water in swimming pools adversely affect my child’s eyes. I give my permission for my child to wear goggles during swimming lessons and accept responsibility should my child be injured as a result of wearing goggles. *YesNo

I am aware that my child will not be allowed to wear goggles for specific water or diving activities for safety reasons. *YesNo

Consent:

· I understand the nature of the education swimming sessions and consent to the participant engaging in the sessions.

• The participant understands that they must behave responsibly at all times and follow instructions from school and swimming pool staff.

• I and the participant understand that earrings (studs are acceptable) and any other form of jewellery including watches must not be worn to the swimming session.

• If I deem it to be appropriate, I will provide my young person with swimming goggles. I and the participant understand that the goggles are their responsibility at all times.

• The information I have provided in this form is accurate at the time of signing and I agree to inform the school as soon as possible of any changes that occur during the swimming programme.

Please note - your child can only be excluded from school swimming on medical grounds as this is a part of the PE National Curriculum
